Bill increase & out of contract refund

I have two separate queries that I require assistance on. Firstly, for my July and August bills, I seem to have been billed incorrectly from £25.89 (new rate since CPI), to £28.12 for the month of July and £31.92 for August.

Additionally,  I have been billed £45.80 for September as an out of contract charge. I received zero notification regarding the contract coming to an end, hence my surprise on seeing this month's bill! Given the circumstances, and my intention to move to another service provider in the soon, what is the process in seeking a refund?
