Not sure I do as it goes from one extreme to the other in the "blame game". No doubt there is a suitable rewording that is fair all round.

After all it was the housemate's incorrect request that triggered this, but a string of cock-ups in BT and PN were what made it go so horribly wrong.

I hope Plusnet have taken a very close look at this sign up page with a view to making some changes that would stop things like this from happening accidentally in future.
One question that does come to mind in this case is if there are 6 lines going into that house, why did it come up with this one and not one of the other 5
